Description

This session will introduce issues related to assessing people's end of life care needs through a proxy when the individual is not able to communicate directly with the person carrying out the assessment.

Objectives

By the end of this session you will be able to:
• Discuss the different types of situation where you may need to use a proxy in assessing someone's end of life care needs
• Recognise the different types of proxy that you can use when making end of life care assessments
• Analyse the challenges surrounding the use of proxies for assessing end of life care needs
• Apply strategies to use proxies appropriately in your practice

Clare is a Macmillan Consultant in Palliative Medicine at Barts and the London NHS Trust and Newham University Hospital Trust, Consultant for 16 years working in areas with high ethnic diversity and deprivation. Interests include communication skills training, clinical leadership in cancer and palliative care and governance. Clare ia slso a member of board of management St Joseph’s Hospice, Hackney.

Pat works as an RGN, RCNT. Macmillan Palliative Care CNS at Barts and The Royal London NHS Trust with a special interest in Head and Neck end of life care.
