This session is about the benzylisoquinolinium neuromuscular blocking agents. The session also compares the currently available benzylisoquinoliniums with the ideal muscle relaxant, and considers the relationship between structure and the pharmacological properties of these drugs. Finally, the session highlights the cardiovascular side-effects of these drugs and how they perform under deranged physiological conditions.


By the end of this session you will be able to:
• Name the benzylisoquinolinium neuromuscular blocking agents available in the UK and identify the intubating dose, onset time and clinical duration of action of each
• Recall how atracurium, mivacurium and cisatracurium are eliminated and how the elimination of these compounds depends on their chemical structure
• State the relationship between atracurium and cisatracurium
• Recall the relationship between potency and onset time
• Identify how the benzylisoquinolinium drugs perform under deranged physiological conditions and list the cardiovascular side-effects and propensity for histamine release by these drugs

Charlene Grassmann -
Charlene has recently been appointed as a Consultant Anaesthetist at the Liverpool Women’s Hospital after completing her training in the North West. She graduated from the University of Stellenbosch in 1997. Areas of particular interest include obstetric anaesthesia and the management of the difficult airway. She also enjoys teaching.

Chris Parker -
Consultant in Anaesthesia
Chris trained in Anaesthesia in Liverpool. He worked with Professor JM Hunter on the pharmacokinetics of atracurium.
He is interested in Postgraduate Education and Training, and the use of simulation for training in crisis management. He has served as College Tutor and as a Royal College Primary FRCA Examiner. He is currently Programme Director for Core Anaesthetic Training in the Mersey Deanery.
