A session that explores the importance of an accurate and contemporaneous anaesthetic chart. Additionally, the role of record keeping in audit, consent and medical legal matters is discussed.


By the end of this session you will be able to:
• State the mandatory variables that should be entered on all anaesthetic charts, according to Royal College of • Anaesthetists guidelines
• Recognize the value of clear, accurate and legible data entry as a medical legal record of events
• Describe the significance of the anaesthetic chart during the handover of patient care between physicians
• Explain the role of documentation in issues of consent and advanced directives
• Describe the importance of accurate and timely record keeping in audit

Ruairi Moulding and Yogen Amin

Ruairi Moulding -
Ruairi Moulding is a Specialist Registrar in Anaesthetics & Critical Care with the Central School of Anaesthetics rotation in London. His special interests include regional anaesthesia. Currently Visiting Instructor in Anesthesiology at University of Michigan, USA.

Yogen Amin -
Yogen Amin BSc, MB ChB, FRCA is a consultant neuroanaesthetist and neurointensivist at The National Hospital for Neurology and Neurosurgery, Queen Square and an honorary senior lecturer at University College London and The Institue of Neurology. His main interests include anaesthesia for epilepsy and awake craniotomies, research ethics and medical education at both undergraduate and postgraduate levels.
