This session looks at the importance of follow up assessments and evaluation of outcomes in end of life care. It also looks at how the actions identified as part of the assessment process help to meet patients’ needs.

Sarah Yardley is a Specialist Registrar in the North Western Deanery. She is currently undertaking a Ph. D. in Medical Education at Keele University in addition to her Palliative Medicine training. She has a particular interest in workplace learning and hopes to continue teaching alongside clinical practice throughout her career.
